Specific Information Form) upon completion of the online application.

The following documents must be submitted along with your Confirmation Receipt:

• Educational Certificates
• Official Transcripts (not applicable for UWI Graduates)
• 2 References
• Birth Certificate
• Legal Affidavit or Deed Poll if your present name is different from that on the Birth Certificate
• Marriage Certificate
• TOEFL Scores (non native speakers of English must score at least 500 in this exam)
• Programme Specific Information Form

These documents must be sent directly to Arthur Lok Jack Graduate School of Business, Max Richards Drive, Uriah Butler Highway North West, Mt. Hope, Trinidad. When submitting documents in person, please bring the originals for verification purposes. If sending the documents via mail, please submit certified copies of all certificates and send to the attention of the Manager, Academic Administration, Arthur Lok Jack Graduate School of Business.
